Health Monitoring Service for Older People – Bay of Islands Hospital |

The arm of this service is to assist older adults with chronic and complex health conditions to remain well and as independent as possible in their own homes. Community Gerontology Nurses visit clients on a regular basis to monitor their health and wellbeing. Problems are identified and action taken to prevent long term problems, hospital admission or rest home placement. Regular liaison is required with GPs and other service providers.
Anyone can refer to the Health Monitoring Services, however a Needs Assessment by our NASC agency must first be completed.

For those living outside the toll free calling area of Whangarei, we have a 0800 number to enable ease of access to our services.
The service is aligned in all the regional hospitals with the following services under the auspices of Services for Older People:
Needs Assessment Service Co-Ordinator (NASC)
Psychiatry for Older People
Hours: Whangarei – Monday to Friday 8am to 4.30pm. Closed on all public holidays.
All other areas – 1-3 days per week, depends on area.
