Patient Information Release

The Release of Patient Information service provides patients, ACC and other requestors with copies of patient records as required by the Privacy Act.

Request forms are available from the main hospital reception desk or can be mailed out on request.

Once completed return to the Clinical Records Department.
Hours:
Monday to Friday: 7.30am – 5.00pm
