The service provides care for frail elderly people over 65 years of age.
Referrals received from Needs Assessor Service Coordination (NASC), District Nurse, GP or social workers
Patients are monitored fortnightly or monthly in their own homes
Hours: 16 hours per week, usually Mon and Tues
