The service monitors frail elderly persons over 65 years, living alone and have a medical condition that requires monitoring
Patients are referred through their hospital, GP, or health professionals working in the community
Assessment is completed by an experienced Registered Nurse
Hours: Mon & Tue 16 hrs per week
