Hospital link to Social Workers and community health services

Social Work services provided include:
  • Meals on Wheels
  • Information on home support in services
  • Supportive counselling (personal)
  • Crisis intervention
  • Financial advise / support, transport / accommodation
  • Advise / information to community resources
  • Advise on Enduring Powers of Attorney and Court Applications under the PPPR Act
  • Elder abuse / neglect assessment / follow-up
  • Discharge Planning
  • Child abuse / neglect assessments / follow-up (NB: All child protection investigations are carried out be the CYPF services)

Patients are referred to this service through their hospital, a GP or other health professionals working in the community.

Assessment is completed by trained Needs Assessors

Hours: 8:30am – 4:30pm Mon – Thurs
