Advise on Enduring Powers of Attorney and Court Applications under the PPPR Act

Elder abuse / neglect assessment / follow-up

Discharge Planning

Child abuse / neglect assessments / follow-up (NB: All child protection investigations are carried out be the CYPF services)
Patients are referred to this service through their hospital, a GP or other health professionals working in the community.
Assessment is completed by trained Needs Assessors
Hours: 8:30am – 4:30pm Mon – Thurs
