Community Rehabilitation Service

The Community Rehabilitation Service is for people in the Mid North community aged 16 years and over who have had a recent medical event such as a  stroke, an amputation, or any other medical condition that has caused a change in how they manage day-to-day life.

The team includes a nurse, physiotherapist, and occupational therapist who can visit you in your home to develop a rehabilitation programme with achievable goals. The team can also refer you on to other services such as social work, dietics, and speech language therapy as needed. These services may be able to see you at your home, or at the Bay of Islands Hospital.

Usually the service is accessed via referral from a health professional, or a hospital if you have had a recent medical event that required hospitalisation.

Each member of the team works 2 days per week all between Monday-Thursday, with no staff in the office on Fridays.
