The Community Rehabilitation Service is for people in the Mid North community aged 16 years and over who have had a recent medical event such as a stroke, an amputation, or any other medical condition that has caused a change in how they manage day-to-day life.
The team includes a nurse, physiotherapist, an occupational therapist who can visit you in your home to develop a rehabilitation programme with achievable goals. The team can also refer you on to other services such as social work, dietetics, and speech-language therapy as needed. These services may be able to see you at your home, or at the Bay of Islands Hospital.
Usually, the service is accessed via referral from a health professional, or a hospital if you have had a recent medical event that required hospitalisation.
Each member of the team works 2 days per week all between Monday-Thursday, with no staff in the office on Fridays.
