COVID-19 Vaccination bookings

Use this form to book a COVID-19 vaccination at one of the Northland DHB vaccination clinics. 

After your first vaccination, remember to book your second vaccination after 21 days after receiving your first vaccination.

When attending your second vaccination,  please remember to bring along your vaccination card.

If there are no free bookings, you have the option of booking a Northland DHB car for up to 1.5 hours to visit a local community clinic for your vaccination.   More information about this can be found at  COVID Pool Vehicles | Northland DHB
