Diabetes Clinical Nurse Specialist

Provides specialty nursing knowledge for clients with diabetes in all age ranges, also diabetes in pregnancy. It is a specialised service and clients need to meet the criteria to be referred.

• Clinical advice, information and education to clients, families/ whānau and staff.
• Clinical leadership and be a resource for the nursing team and other staff within DHB and other providers.
• Work with other providers to have coordinated care for clients.
• Work within a multi-disciplinary team for best outcome and management of clients.
• Run nurse-led clinic and home visiting.
