Access your healthcare information

 

Free access to your personal healthcare information; whenever and wherever

Online patient portals are an easy, convenient, safe and secure way to access your key health information; anytime, anywhere.

Te Whatu Ora in Te Tai Tokerau Northland will soon be able to send your future hospital care documents and notifications to you via the Manage My Health portal.  To request your previous hospital patient records, visit Release of Patient Information Service 

 

                 

 

To access your hospital care documents and notifications

  • If you have a Manage My Health account through your GP (or other community healthcare provider), you will receive an email and text to notify you when your hospital care information becomes available to view. This is scheduled to happen over the second-half of August 2023.
     
  • If you don’t have a patient portal account through your GP, enquire with your GP if they offer Manage My Health.  If they do, simply ask to register and then activate your account.  This will also enable you to access your hospital care information.
     
  • If your GP uses another patient portal, such as Health 365 or ConnectMed, you can still have Manage My Health to view your hospital care information. Next time you are at Whangārei Hospital, ask a reception staff member to register you with the Manage My Health hospital patient portal.
     
  • If you are not enrolled with a GP, you can still have a Manage My Health account to view your hospital care information. Next time you are at Whangārei Hospital, ask a reception staff member to register you with the Manage My Health hospital patient portal.

   

To access your primary care information

Simply ask at the reception of your GP or other community healthcare provider about registering for the patient portal they use (e.g. Manage My Health, Health 365 or ConnectMed) to give you online access to your health information such as:

  • health conditions
  • allergies
  • prescriptions
  • immunisations
  • lab results
  • clinician notes
  • recall due dates
  • hospital clinic letters, discharge summaries and referral updates, via the Manage My Health portal (details above)

Some also let you interact with your GP or community healthcare provider to:

  • book appointments
  • request repeat prescriptions
  • message your GP

Check with your GP or community healthcare provider to find out what functions they offer.
 
 

Other key benefits patient portals provide include:

  • See progress of your referrals to other healthcare services. Receive a notification when your referral by a GP or community healthcare provider is received by the hospital service.
     
  • Access and share your health information with a hospital service or commuity healthcare provider outside of your home district or region or if you are overseas. 
     
  • Retain access to your health information if you move to a new house or move to another district or region and/or enrol with a new GP or community healthcare provider.
