Mask Fit Testing 

Use this form to book in for a Mask Fit Test through Northland DHB Sites. 

Mask fit testing assesses whether the wearer is obtaining a good seal between their face and the mask and minimising their risk of aerosol biohazard exposure.

You will be provided with details of the successfully fitted mask make and model at the time of testing.

When you come to your appointment, can you ensure you do the following:

• Be clean-shaven
• Do not eat, drink, chew gum or smoke for at least 15 minutes before your test
• Bring the clean respirator and any filters you normally use (if applicable).
